Prompt Reporting
BETA encourages the prompt reporting of incidents, adverse events, and claims, enabling timely assessment and resolution that benefit patients, employees, and healthcare organizations alike.

When do I contact BETA?
For Professional Liability Claims:
If members receive any of the following documents, contact our Claims Department immediately for assistance:
- Notice of Intent to Sue or Notice of Intention to Commence Action
- Written claim or demand
- Letter from an attorney
- Claim against a Public Entity
- Lawsuit (summons or complaint)
- DFEH complaint
- EEOC charge
- Subpoena for deposition or trial
- Any relevant legal papers that are confusing or difficult to understand
Members should also contact us as a precaution if any of the following events occur, so the Claims Department can fully investigate both the existing claims and incidents likely to become claims:
- An unexpected complication resulting in death or serious injury to a patient
- “Errors” by an employee resulting in death or serious injury to a patient
- A letter from a patient (or an attorney) objecting to payment of a bill because of complaints about the care provided
- A threat of litigation (oral or in writing) made by a patient, family member or attorney
When reporting any of the above, please include the name of the potential claimant, the date of the incident and a brief description of the incident.
For Workers’ Compensation Claims:
When an employee notifies the healthcare facility of injury, crucial initial steps must be taken. The employee should be directed to obtain appropriate medical care at your appointed occupational clinic, employee health clinic or other designated facility. The law requires that an employer provide any employee claiming industrial injury with a Claim Form (Form DWC-1) within one day of notice of injury, even if the employer disputes the injury. The law permits delivery of Form DWC-1 by hand, mail, or electronically.
When the injured worker completes the Claim Form DWC-1, please submit it to BETA with the Employer Report of Occupational Injury (Form 5020) and any other pertinent information.
How do you report a claim?
In the event of a formal claim or a precautionary incident, healthcare facilities and their personnel need to be protected. Claims can be submitted online, emailed, mailed, or faxed to BETA.
